As the benefits of doula support during childbirth and postpartum become more widely recognized, an increasing number of insurance companies are beginning to offer reimbursement for these services. However, the process can be complex due to the lack of standardized billing codes for doulas.​

Current Procedural Terminology (CPT) codes are used by healthcare providers to describe services rendered for billing purposes. While there isn't a specific CPT code exclusively for doula services, some codes are commonly used:​


  • 99499: "Unlisted evaluation and management service." This is a general code sometimes used for labor support, but it's important to note that it's not specifically designated for doula services and is intended for licensed medical professionals 

  • 99501: "Home visit for postnatal assessment and follow-up care." This code is used for postpartum visits but is typically associated with services provided by registered nurses or other licensed healthcare providers

  • 99502: "Home visit for newborn care and assessment." Similar to 99501, this code is generally used for services rendered by licensed professionals 


It's crucial to understand that using these codes for doula services can be problematic, as they are not officially designated for non-medical support roles like doulas. Misuse of these codes may lead to claim denials or potential legal issues.​


The Importance of NPI Numbers


A National Provider Identifier (NPI) is a unique 10-digit identification number issued to healthcare providers in the United States by the Centers for Medicare & Medicaid Services (CMS). While doulas are not licensed medical providers, obtaining an NPI number can be beneficial when clients seek reimbursement from insurance companies 

To apply for an NPI number, doulas can visit the National Plan and Provider Enumeration System (NPPES) website. Having an NPI number may facilitate the reimbursement process, but it's not a guarantee of coverage.


Steps to Seek Insurance Reimbursement for Doula Services


  1. Pay Your Doula in Full: Most insurance companies require that services be paid upfront before considering reimbursement.​

  2. Obtain a Detailed Invoice: The invoice should include:

    • Doula's name and contact information

    • NPI number (if available)

    • Tax ID or Social Security Number

    • Dates and descriptions of services provided

    • Applicable CPT codes (with caution, as noted above)

    • Diagnosis codes, if applicable

    • Total charges and payment confirmation​

  3. Submit a Claim to Your Insurance Company: Include the invoice and any required claim forms. Be prepared for initial denials, as doula services are not universally covered.​

  4. Provide Additional Documentation if Necessary:

    • A letter from your doula detailing their qualifications and the services provided

    • Letters from healthcare providers (e.g., obstetricians or midwives) supporting the necessity and benefits of doula care

    • Personal statement explaining the value and impact of doula support during your pregnancy and childbirth​

  5. Appeal if Denied: If your claim is denied, don't be discouraged. Follow up with your insurance company, provide additional documentation, and consider appealing the decision. Persistence can sometimes lead to successful reimbursement


Alternative Payment Options


If insurance reimbursement proves challenging, consider the following alternatives:​

  • Health Savings Accounts (HSAs) or Flexible Spending Accounts (FSAs): These accounts often cover doula services, allowing you to use pre-tax dollars for payment

  • Employer Benefits: Some employers offer wellness benefits or programs like Carrot Fertility that may reimburse doula services


Final Thoughts


While insurance reimbursement for doula services is not guaranteed, understanding the process and being prepared with the necessary documentation can improve your chances. Always communicate openly with your doula and insurance provider to navigate the reimbursement landscape effectively.​
